New Patients

Welcome to Rocky Mountain Dermatology! We are excited to have you as our guest. There are a couple of things we'll need from you during your first visit to help us get acquainted and best understand your needs:

Please arrive 5-10 minutes before your scheduled appointment time to allow us to review new patient paperwork and enter in your insurance information. Bring a photo I.D. and current insurance cards to your visit. If your insurance requires a referral from a Primary Care Provider, please have the referral sent over to our office prior to your appointment. 

We have two required forms for new patients (three if the patient is a minor). These forms are available below if you would like to fill them out prior to your visit and save some time checking in. They are also available in the office. The "New Patient Registration" form will include basic information about yourself, including your contact information. The "Financial Policy" outlines our policies requiring payments and insurance claims. If you plan on attending your visit via Telehealth, or online video conferencing, please fill out the required forms and return them to the office via email as directed. If you have any questions about either form, please give us a call and we'll be happy to go over them with you!

If the patient is a minor, they must be accompanied by a parent or legal guardian. If the legal guardian is unable to attend the visit, they may fill out the "Authorization to Treat in Absence of Parent or Guardian" form included below. This form allows the parent or guardian to give permission to another responsible adult to attend their child's treatment in the parent's absence (this could include a grandparent, adult sibling, friend of the family, etc.). There is also an option for minors 16-18 years of age to be seen by themselves with parent permission. In order for a minor patient to be seen without their parent or guardian, either in the presence of another adult or by themselves, the proper form must be signed by a parent or guardian and returned to the office before the patient's appointment. 

To help us best serve you during your appointment, please consider bringing the following with you:

- Questions or concerns that you would like to discuss with your provider. Writing it down is a good way to remember all of your questions and ensure they all get answered during your appointment!

- A list of current medications, allergies, or medical conditions.

- Any family history of skin cancer, such as Melanoma.
